    1. Original copy of BACC II (G12 national examination) certificate.
    2. Four (4) copies of 4" x 6" photo
    3. Accomplished Application Form and Registration Fee.
    4. Qualifying Entrance Test
    5. Transcript of Records from High School (if any)

  2. Foreign Students
    1. Original High School Diploma (with English translation, if not written in English Language)
    2. Original Alien Certificate of Registration (ACR) issued by the Ministry of Foreign Affairs or passport.
    3. Personal History Statement
    4. Accomplished Application Form and Admission Fee (USD 15.00)
    5. Six (6) copies 4" x 6" photo.
    6. Transcript of Records (with English translation)
    7. A "Passing Mark" of English Language Test (In case of poor English Language Comprehension, student shall be required to enroll at Norton's Language Institute for Special English Class)
    8. Medical Health Certificate, with standard size chest X-ray.
